Add "Testimonial Form" (Divi Module) On Divi Page

  1. Now that you are ready to collect testimonials from your customers, it's time to add a testimonial form to a page. To do so,

    • Create or edit a page in the divi visual builder
    • Within a divi row click on the plus icon for Add New Module
    • Search for Testimonial Form, click and add the divi module.

Testimonial Form Settings (Content Tab) - Divi Testimonial Form

  1. To associate one or more pages to the testimonial form, show or hide input elements in form

    • In the Testimonial Form divi module, goto Content tab.
    • Click on Testimonials Form. Setting options are
      • Associated Page(s) - Links one or more pages to the submitted testimonial. Options are
        • Current Page - Links the testimonial to the custom page
        • Custom Pages - Links the testimonials to set of specified page IDs.
          • Custom Page IDs (comma-separated) - Add the comma-separated page IDs that associates with the testimonial.
      • Show Rating Stars - Toggle YES to allow customers to set a star rating on the form
      • Show Title - Toggle YES to allow customers to add a title to the testimonial
      • Show Content - Toggle YES to allow customers to add content to the testimonial
      • Show Name - Toggle YES to allow customers to add author name to the testimonial
      • Show Email - Toggle YES to allow customers to add an email to the testimonial
      • Show Consent / Terms Field - Toggle YES to show the consent field for terms and conditions
